At Brookfield Dentistry we understand that each financial situation is unique. We provide a number of financial options to work with your budget and help you and your family receive the care you need. Our office accepts many dental insurances. Please contact us to verify that we accept yours.

We also work with CareCredit and WDA Savings Club to offer flexible payment options for you. These are third-party financing groups which offer low- and no-interest financing. You can learn more about these groups by following the links provided below.

We offer a 5% discount for payments made out-of-pocket on the day of service. This discount is only available if you are paying with cash, check or credit card.

Brookfield Dentistry Dental Membership Program

No insurance? No problem! We offer an in-house membership plan to help you receive the quality dental care you need at an affordable price.

Our dentists and team have created this plan to give you immediate access to dental care – with absolutely NO deductibles, waiting periods, maximums, claims forms, calling insurance companies, pre-determinations, pre-existing conditions or open enrollment periods!

We offer several plan options, including:

Terms and Limitations

• Plans will renew each year unless a termination request is received in writing. We will send a statement when your plan is due for renewal. If your plan is terminated, re-enrollment within two years will incur an additional $40 fee.
• Payment is due in full at registration and upon yearly renewal date. Fee is non-refundable unless there is an unforeseen circumstance such as relocation or death, and refunds are at the sole discretion of our office.
• Plans are not transferable.
• All products are excluded from your plan.
• Payment for services rendered is due in full on the date of service. Failure to pay on the date of service will result in forfeiture of the discounted rate. Payment plans may be arranged with our team for orthodontic treatment.
• Rates are subject to change annually. You will be informed in writing of any rate changes 30 days prior to plan renewal.
• Services do not accumulate or roll over from year to year. Benefits or services left unused will be forfeited.
• This plan is valid only at Brookfield Dentistry. Membership does not transfer to outside doctors or specialists.
• The 15% discount does not apply to orthodontic treatment, including replacement retainers, implant treatment and implant-related services, BOTOX® treatment, teeth whitening treatment, Invisalign® clear aligners or Reveal orthodontic treatment.
• CareCredit cannot be used to pay for membership plans. If you elect to pay using CareCredit, your 15% discount will not be applied.
• Current patient accounts must be in good standing, with no past due balances.
• Patients must follow all Brookfield Dentistry policies. Cancelled appointments with less than two business days’ notice may incur a $50 fee. This must be paid prior to utilizing plan benefits, including discounted procedures.
• Members may be dismissed from the membership program after two cancelled appointments or for nonpayment. Enrollment fees are non-refundable.
• This is not an insurance plan and cannot be used in place of or in combination with insurance. It is the patients’ responsibility to notify Brookfield Dentistry of any changes to their insurance status.
